"Rider" or legal guardian of "rider" is required to complete this form prior to boarding the bus and agrees to the following terms:

• Fly-Rides requires all participants to be over the age of 18 or have express permission from their parent or guardian.
• No hanging from the ropes inside the bus.
• No moving while it is in motion; you do so at your own risk.
• The bus driver reserves the right to remove disruptive passengers at their sole discretion for the safety of those on board as well as other travels on the road. If you are removed from the bus you are responsible for any further transportation costs.
• Do not leave any personal items on the bus unattended. Fly-Rides is not responsible for any lost, damaged or stolen items during the ride or left on the bus unattended.
• Fly-Rides is not responsible and will not reimburse or compensate Rider for unexpected vehicle breakdowns, accidents, severe weather, traffic delays, or any act of God that may delay the trip or cause cancellation of the trip.
• If loading zone and/or route is deemed unsafe by Fly-Rides or its employee(s), drivers have sole discretion for alternative loading/unloading/route.
